It reads Institut du radium de Montréal - Admission card. The Name field is filled in with Mme. Édouard Bolduc, and the Address field with 1462 Letourneux. The card ends with Please - Keep this card and always bring it to the Institute.

(Top) Mary Travers, better known as “La Bolduc,” was a famous Québec musician and singer.

(Bottom) This card was issued by the Radium Institute to Mrs. Bolduc during her cancer treatments. To the best of our knowledge, this is the only existing representation of one of the Institute’s admission cards. The document includes the patient’s information: patient number, attending physician, name and address in Montréal (more specifically in the Maisonneuve neighbourhood).
